The nuclear bombs in Fallout are a core part of the series’ lore, but the mystery of who started the nuclear apocalypse is even more of a twist. As Fallout explained in its eighth episode, Vault-Tec, the company that created 122 vaults to shelter paying customers in the case of nuclear war, is responsible for starting that war in the first place.
